Registrar forms for faculty and advisors

Section Change Request Form for department chairs to make changes to course offerings already live on Arches. 

Department Chair Substitutions Request Form to submit substitutions and waivers for student programs as approved by the department.

Exam Week Forms for multi-section requests and group travel 

Grade Change Request Form Faculty will receive an automated email response a day after if grades are approved. Grades not approved will result in an email for AS&D. 

Program Change Form This form is used for changing advisors.

Add/Course Permission Form form is used for courses requiring instructor permission, closed sections, independent/directed study and departmental approved pre-enrollments.

Drop Form for advisors to approve/deny drop requests and instructors to add last date of attendance.

Administrative Withdrawal Request Form for faculty seeking to request a student with excessive absences be removed from their course.

Departmental Oversight Access Request form - request a member of an academic department to receive security access to departmental rosters and course offering information in Arches

Course Cap Variance Form - for a department chair to request a change in the standard course cap policy for a particular section. 

Incomplete Agreement Form - only use this form when you have assigned an incomplete for the semester.

List of FYI-102 approved substitutions - students and advisor should carefully read the information about FYI-102 and their catalog requirements in the Degree Requirement section of the catalog. 

Data Requests - The Office of the Registrar is responsible for maintaining individual and aggregate student data, as well as institutional data on published catalog procedures and policies, grading, transcripts, curricular offerings, degrees, eligibility, academic progress, honors, and awards, among many others. The institution makes many reports available to the public on our Institutional Research webpage. As a private institution, Augustana does not participate in public record requests beyond what is published on the IR site. Internal departments and offices needing data should first consult reporting access in Arches under the "My Reports and Links" button. Those who need additional information in order for them to complete their institutionally-defined job duties may request a report or assistance with reporting by sending an email request with detailed requirements to registraroffice@augustana.edu. Please provide a lead time of a minimum of 10 business days for the request (additional time may be needed during peak times at the start or end of a term or surrounding breaks). Some data may not be released if access to the data is determined not to be required for an individual to perform their job duties and/or if it is not in the best educational interests of the student. All institutional privacy and FERPA policies apply to the release of protected information.
